Lekowrażliwość patogenów wywołujących zakażenia układu moczowego u dzieci z odpływami pęcherzowo-moczowodowymi – doświadczenia własne ośrodka

Abstract Background Vesicoureteral reflux (VUR) is the most common anatomical malformation in children with urinary tract infection (UTI). The reflux nephropathy is one of the major causes of chronic kidney disease in children and adolescents. Recurrent UTI in patients with VUR may result in variability of uropathogens and their drug sensitivity. The aim of study was to estimate uropathogen drug sensitivity in children with VUR hospitalized in 2009–2013 in Department of Pediatric Nephrology, Medical University of Lublin. Materials and methods The study included retrospective analysis of medical records of 54 children aged 9 months to 17 years (median 5.6 year) with VUR treated due to UTI. The studied patients were divided into two groups: A (61.1%) – patients with I–III grade VUR and B (38.9%) – patients with IV–V grade VUR. Results In children with VUR 134 (31.5%) episodes of UTI (41% in group A and 50% in group B) were reported, respectively. The significant differences of species of isolated uropathogens between groups A and B were observed. In both analyzed groups, Escherichia coli was the predominant pathogen, but with significantly lower incidence as UTI causative agent in group B. Additionally, the significant differences of drug sensitivity in analyzed time period were revealed. Conclusion High variability of isolated uropathogens and their different drug sensitivity detected in children with high grade VUR should be considered in empirical treatment of UTI in this group of patients.
