“Splayed distal right coronary artery sign” in infero basal left ventricular aneurysm

Abstract A middle-aged gentle man had presented with inferior wall myocardial infarction, for which he had undergone primary PCI. A year later, he had presented with exertional angina. He developed ventricular tachycardia during stress testing. Coronary angiogram showed a splayed distal right coronary artery. Left ventricular angiogram showed postero-basal aneurysm. Patient underwent coronary artery bypass grafting, left ventricular aneurysmectomy and intra operative radiofrequency ablation.
