A case of Behcet's disease myocarditis documented by cardiac magnetic resonance imaging.

Abstract Acute myocarditis is extremely rare in Behcet's disease (BD). Here we report a case of a 32-year-old man with a history of oral ulcerations who was admitted for fever and chest pain. On examination, he had bipolar ulcers with pseudofolliculitis and a positive pathergy test. Electrocardiogram and transthoracic echocardiography were normal. Cardiac magnetic resonance imaging (MRI) revealed left ventricular inferior and lateral wall late gadolinium enhancement (LGE). A diagnosis of acute myocarditis revealing BD was made, and the patient was treated with colchicine and pulses of methylprednisolone, and cyclophosphamide. A 7-month follow-up MRI showed a substantial decrease in LGE area.
